Program: Family Support Team, Alma, Community Liaison

Reports to: Director of Programs and Innovation

Schedule: This is a 40-hour per week position, Monday through Friday. This position requires availability during programming hours from 8:00 am – 6:00 pm. Some evening and weekend availability required. Times are subject to change based on programming needs.

Salary Range: $60,000 – $85,000, based on qualifications and previous related work experience.

Job Description:

Our dedicated Family Support Team offers wraparound services to all Valley Settlement participants and the community. Family Navigators conduct home visits, assist families in times of crisis, and provide important one-on-one support to connect families with the resources they need to thrive.

Peer compañeras, who are trained in peer mentoring, provide a peer-to-peer mental health support program for mothers who are going through post-partum depression.

Family Support Team

Duties and Responsibilities include, but are not limited to:

  • Ensure successful programming for the Family Support Team (FST) by meeting organizational and programmatic goals, and grant deliverables
  • When possible, use a collaborative leadership style when making programmatic decisions
  • Supervise and support all team members through one-on-one supervision meetings and  regular observations in the field
  • Plan monthly and yearly calendars for staff members including scheduling deadlines, natural pauses, visits to all programs, trainings, and other events
  • Complete annual staff evaluations per the organization’s performance review process
  • Hold weekly team meetings for FST
  • Attend and participate in leadership meetings
  • Attend partner meetings and workshops relevant to the work
  • Build strong relationships with the other program managers and ensure communication is effective and timely
  • Coordinate activities and events in partnership with program managers
  • Accompany the Community Liaison to community events to provide support and assist with children/families/community where needed
  • Accompany the Navigators on home visits, when requested, to provide support and assist with observations of children/families where needed
  • Review and revise family work plans, as needed, in collaboration with the Navigators
  • Provide support to families with special questions and concerns
  • Responsible for current knowledge of community resources and updating the FST, and all staff, when appropriate
  • Manage FST’s community resource external partnerships and establish referral processes between VS and the agencies
  • Follow up with referrals to external agencies based on family needs including initial referral if the agency is new
  • Oversee the FST’s internal referral process. Actively manage cross-programming referrals, bringing together the program referral teams to discuss how best to support the family
  • Oversee the FST’s external referral process and lead out on cross-agency referrals that involve one family
  • Maintain supporting and connecting 5 families, at all times, to resources
  • Responsible for maintaining inventory of supplies and ordering materials needed
  • Responsible for onboarding new team members in coordination with Human Resources and other team members
  • Lead out on programmatic training and professional development for the team and support individual team members in their growth plans
  • Collaborate with the Development and the Finance and Operations teams to develop program budgets and maintain clear communication on programmatic changes or needs; complete grant deliverables and provide Development with program data and narrative for grant reports
  • Coordinate and follow all necessary survey/data collection protocols including following timelines and training the team when necessary
  • Ensure integrity of program-related data in the program database through weekly database reporting from the team to ensure accurate information is being entered, including household and participant information, photo release documentation, home visit notes, and family work plans
  • Keep Director of Programs & Innovation up to date on all aspects of programming through weekly meetings, monthly reports, and other communication as necessary
  • Attend program tours, as needed, to represent FST to funders and partners
  • Lead in door-knocking efforts for recruitment and potential information sharing with the other program managers
  • Implement and oversee the Promotora volunteer model pilot program (to start in January 2023)
  • Other duties, as assigned

Alma

Duties and Responsibilities include, but are not limited to:

  • Ensure successful Alma programming (mental health programming) by meeting organizational and programmatic goals, and grant deliverables
  • When possible, use a collaborative leadership style when making programmatic decisions
  • Support Alma peers in their peer support groups to ensure they are successful
  • Supervise and support team members in weekly meetings
  • Complete annual staff evaluations per the organization’s performance review process
  • Accompany Alma Peer on home visits, when requested, in order to support staff and assist with observations of children/families where needed
  • Lead trainings or group sessions with the team on mental health and be available to families with special questions and concerns
  • Perform initial intakes, the end of program assessment and 6-month post assessment for the Alma participants
  • Manage internal and external mental health referrals
  • Work with outside consultants who provide behavioral activation support for the Alma Peers
  • Coordinate and follow all necessary survey/data collection protocols including following timelines and training the team when necessary

  • Other duties as assigned

Minimum Requirements:

  • Bilingual (English and Spanish, oral and written)
  • Bachelor’s degree in Early Childhood, Education, Social Work, or other related field or equivalent work experience
  • Experience working in mental health settings
  • Experience working with children and families
  • Experience training staff members
  • Knowledge of early childhood development preferred
  • Experience with collecting data and assessing of data
  • Proficient in Microsoft Office Suite and Google Drive (Documents & Spreadsheets)
  • Have and maintain a valid Colorado driver’s license
  • Must have a reliable vehicle for field work and to attend external meetings (mileage reimbursed)
