Reports to: Director of Programs & Innovation

Schedule: This is a 40-hour a week, non-exempt position, Monday through Friday. This position requires availability during programming hours from 9:00 am – 5:00 pm, with some evening availability required. Times are subject to change based on programming needs.

Salary Range: $24 -$31 per hour, based on qualifications and previous related work experience

Job Description:

The Program Family Navigator is responsible for the successful implementation and operation of the Family Support team’s referral-based work for all of Valley Settlement’s programs.

Duties and Responsibilities include, but are not limited to:

Program cycle of Family Support Team (activities are ongoing throughout the year):

Planning Phase:

  • Works collaboratively with Valley Settlement (VS) staff to receive family / individual referrals
  • Works collaboratively with community agencies to connect with family / individual referrals
  • Leads in training new and current staff in the referral process
  • Engages with all VS programs to present to programing participants on the role of the FST team
  • Engages in designing program calendar for FST and Promotoras including training dates, evening and weekend events or workshops, and any other activities as instructed by their supervisor
  • Ensures spaces in the community are well maintained when used to meet with families/individuals and are confidential for participants to share their story. If children are present, the space is safely secured through physical security measures and through childproofing.
  • Responsible for participant recruitment for all programs, through various methods such as door knocking and attending events
  • Organizes and maintains the inventory of program materials

Enrollment / Implementation Phase:

  • Conduct the first call / home visit with families / individuals referred to VS and seek to understand the different challenges and priorities they want to act on and develop an action plan with the family
  • Coordinates and facilitates connections between families/individuals and community, state, or national resources according to family’s action plans, providing ongoing follow-up to ensure successful access to services
  • Maintains effective recording system for all referrals, including referral status and referral outcomes; enters referral data into the data tracking system
  • Develops and maintains partner relationships with multiple resources to support the connection made for families/individuals
  • Attends partnership meetings at the local, state and national level engaging in resource navigation work
  • Communicates effectively with staff, outside agencies, families/individuals about referral processes and provides updates on the work that is being done
  • Establish and maintain respectful and trusting relationships with families/individuals, and partners with a focus on a asset-based approach
  • Leads evening sessions for families or weekend workshops for families
  • Completes weekly reports for the supervisor, detailing what tasks were completed, in progress or have not started yet

Family Engagement & Education

  • Create and share resources and educational materials for families/participants
  • Must communicate with families in a culturally responsive and empathetic way
  • Participate in planning, organizing, and facilitating family education events
  • Build trusting relationships with families in our community
  • Participate in planning, organizing, and facilitating program graduations

Program Monitoring and Evaluation Phase:

  • Participates in yearly evaluation retreat and evaluation process which might include pre/post surveys with families or with partner organizations
  • Engages in annual program targets, strategic goals and grant deliverables
  • Turns in reports as needed by supervisor
  • Monitors any grant deliverables tied to that programing year
  • Attends weekly one-on-ones with the supervisor to build a relationship, rapport and to communicate
  • Attend weekly team meetings and support team efforts

Organizational Leadership:

  • Attend events periodically for all VS programs in the community
  • Attend program staff meetings and trainings when scheduled
  • Engages in site visits or program tours as needed by development team
  • Participate in annual door-knocking recruitment efforts for all programs
  • Performs all functions and activities within the guidelines and philosophy set forth in VS’s values, mission, vision, and goals
  • Co-leads in onboarding and training
  • Other duties, as assigned

  Minimum Requirements:

  • Bilingual (English and Spanish, oral and written)
  • High School Diploma or GED required
  • At least two years’ prior working experience in the health and human service field, with direct experience working with families.
  • Proficient in Microsoft Office Suite and Google Drive (Documents & Spreadsheets)
  • Possess and maintain a valid Colorado driver’s license and automobile insurance
  • Available and reliable vehicle to attend external meetings (mileage reimbursed)
